Summer Camp Pre-Screening Health Form
 

 

The Stonewall Jackson Area Council urges all campers and staff to continue taking preventive measures to protect against transmission of the novel H1N1 (Swine Flu) virus.

Everyone attending a summer camp activity (Cub Day Camp, Webelos Resident Camp, Cub Family Camp Weekend) is asked to complete our very brief
"Pre-Screening Form"


All Scouts, family members, volunteers and staff attending any of our summer camps/activities should complete a "Pre-Screening Form for the novel  H1N1 virus.  This form is a checklist of flulike symptoms to alert you of the potential for someone having this virus.  Sample form below.




All Scouts, family members, volunteers and staff attending any summer camp/activity should complete the form prior to arrival at camp/activity. Anyone checking "Yes" to Question #1 AND any other question should see their doctor or health care provider before loading up and coming to camp.
If these symptoms persist before leaving for camp, the person should NOT attend camp.

Please bring this form with your medical and health form paperwork! Our Medical Officer or a staff member will check all persons prior to the camp/activity. If someone arrives at camp without this form, they
will need to complete prior to being admitted...so please bring this completed form with you.

Summary

We want you to have a great time this summer!  It is absolutely true that anyone with flulike symptoms does not automatically have the novel H1N1 (Swine Flu) virus, however, if you are sick, STAY HOME! Parents, if your child is sick, KEEP ’EM HOME!—For at least 7 days after symptoms began or 24 hours after symptoms resolve, whichever is longer.
