| This is the new Annual
Health and Medical Record form. It is replacing the current Class 1, 2
and 3 health forms. The major
differences are -
Everything is now in one place.
It is a fillable, and savable, PDF
form.
The form is valid for one year, however
you may update online as needed.
For activities lasting over 72 hours a
medical evaluation by a licensed health-care provider is now mandatory.
In the medical evaluation section (B)
there is a height/weight chart that MUST be strictly adhered to if the
event will take the unit beyond a radius wherein emergency evacuation is
more than 30 minutes by ground transportation.
This form is for both youth and adult.
